The Children with Special Health Needs (CSHN) program in Fairlee, VT, is dedicated to supporting children and youth from birth to age 21 who have special health needs. Our mission is to provide comprehensive, culturally sensitive, and family-centered services that empower families and ensure access to vital resources. With a dedicated team of medical social workers, nurses, and a variety of specialty providers, we offer care coordination to navigate the complex healthcare system, as well as Children's Personal Care Services, Pediatric Hi-Tech Nursing, Pediatric Palliative Care, a Child Development Clinic, and Community Nutrition Services.
