Located in Franklin, VT, Cancer Transitions is a dedicated program designed to empower cancer survivors after they have completed their treatment. Focusing on individuals who have finished their treatment within the last 24 months, we also welcome those in any stage of survivorship. Our holistic approach includes providing nutritional support, personalized exercise plans, emotional health education, and medical management resources. We aim to guide survivors towards a healthier, more fulfilling life, encouraging a proactive mindset as they transition into this new chapter of their journey.
