The Children with Special Health Needs (CSHN) program in Highgate Center, VT, is dedicated to supporting children and youth from birth to 21 years with special health needs. Our mission is to provide comprehensive, culturally sensitive, community-based, and family-centered services that empower families to navigate complex health systems effectively. We offer a wide range of essential services, including care coordination, access to a team of experienced medical social workers and nurses, pediatric hi-tech nursing, pediatric palliative care, and developmental assessments through our Child Development Clinic. Additionally, our Community Nutrition Services ensure that children receive the nutritional support they need. Through these services, we strive to enhance the well-being of children with special health needs and their families in Vermont.
