Cancer Transitions is a dedicated program located in Hyde Park, VT, designed to support cancer survivors who have recently completed treatment, ideally within the last 24 months, although individuals at any stage of survivorship are welcome. Our evidence-based approach encompasses essential services, including nutritional support to foster healing, personalized exercise plans to enhance physical well-being, education on emotional health management to promote mental resilience, and guidance on medical management for ongoing care. We are committed to empowering survivors on their journey to health and wellness, helping them navigate the transition to living life fully after cancer.
