The Children with Special Health Needs (CSHN) program in Ludlow, VT, is dedicated to supporting children and youth from birth to age 21 who have special health needs. Our mission is to provide comprehensive, culturally sensitive, and family-centered services that empower families and enhance the well-being of their children. With a team of medical social workers, nurses, and specialty providers, we offer critical care coordination and a wide range of services including Children’s Personal Care Services, Pediatric Hi-Tech Nursing, Pediatric Palliative Care, Child Development Clinic, and Community Nutrition Services.
