The Children with Special Health Needs (CSHN) program in New Haven, VT, is dedicated to supporting children and youth from birth to age 21 who have special health needs. Our mission is to provide comprehensive, culturally sensitive, and family-centered services that empower families to navigate the complexities of healthcare. With a dedicated team of medical social workers, nurses, and specialty providers, we offer essential services such as care coordination, Children's Personal Care Services, Pediatric Hi-Tech Nursing, Pediatric Palliative Care, a Child Development Clinic, and Community Nutrition Services.
