Located in North Troy, VT, Cancer Transitions is a supportive program designed specifically for cancer survivors who have recently completed treatment. Although primarily aimed at those within 24 months of finishing their treatment, individuals at any stage of survivorship can benefit from our comprehensive offerings. We focus on empowering survivors through nutritional support, personalized exercise plans, emotional health education, and guidance on medical management. Our mission is to enhance the quality of life for cancer survivors, ensuring they have the resources and assistance needed to thrive during their journey.
