Based in Pittsford, Vermont, Cancer Transitions is dedicated to empowering cancer survivors who have recently completed their treatment, especially those within the last 24 months, although support is available to all survivors at any stage of their journey. Our evidence-based program focuses on comprehensive care, offering vital resources such as nutritional support, tailored exercise plans, emotional health education, and medical management guidance.
