Located in Proctor, Vermont, Cancer Transitions is a supportive program designed for survivors of any cancer diagnosis who have completed their treatment. While primarily focused on individuals who have finished treatment within the last 24 months, the program is open to all survivors seeking assistance at any stage of their recovery journey. We provide crucial services including nutritional support, customized exercise plans, emotional health education, and medical management guidance, all tailored to enhance the well-being and resilience of cancer survivors.
