Based in Putney, VT, Cancer Transitions is a dedicated program designed to support survivors of any cancer diagnosis who have recently completed treatment. While primarily focused on individuals who finished treatment within the last 24 months, our resources are valuable to survivors at any stage of their journey. We offer essential services including nutritional support, personalized exercise plans, emotional health education, and guidance on medical management.
