KidneyCare:365, located in Swanton, VT, is dedicated to empowering individuals with the knowledge and resources needed to navigate kidney health and disease. Our program offers comprehensive education tailored for those recently diagnosed with kidney disease, caregivers, or anyone preparing for dialysis. Participants will learn about the symptoms and stages of chronic kidney disease (CKD), nutritional guidance for eating well with kidney disease, coping strategies, treatment options including home dialysis, and a variety of resources to support their journey. We understand that flexibility is key, which is why our classes are conveniently offered in multiple formats: online, by video conference, phone call, or in person. Together, we aim to foster awareness, support, and proactive health management for kidney disease.
