The Children with Special Health Needs (CSHN) program in Underhill, VT, is dedicated to supporting children and youth up to age 21 with special health needs by providing comprehensive, culturally sensitive, and family-centered services. Our compassionate team of medical social workers, nurses, and specialized providers work collaboratively to help families navigate the complex health system effectively. We offer a range of essential services, including care coordination, Children’s Personal Care Services, Pediatric Hi-Tech Nursing, Pediatric Palliative Care, the Child Development Clinic, and Community Nutrition Services.
