Welcome Home Program in Sammamish, WA, is dedicated to supporting individuals recovering from hospital stays by providing 12 weeks of nutritious, delicious meals delivered directly to their homes within 48 hours of referral. Our mission is to ensure that those in recovery receive not only meal assistance but also personalized guidance through sessions with a Registered Dietitian Nutritionist (RDN). We empower individuals on their path to health and well-being by making meals accessible and tailored to their needs. To enroll in our vital service or to refer someone in need, healthcare providers can easily complete our enrollment form and submit it to Lifelong.
