Healthy U is dedicated to empowering individuals in Hulett, WY, through the Stanford University Chronic Disease Self-Management Program (CDSMP). This complimentary six-week workshop is designed for those living with chronic health conditions and their caregivers, offering essential skills and resources to enhance daily living. Participants can choose between in-person and virtual sessions, engaging in topics such as effective problem solving, nutritious eating habits, action planning, emotional awareness, physical activity, treatment evaluation, and communication strategies with healthcare providers. Our mission is to foster independence and improve quality of life for all participants, making health management accessible and supportive.
