The Living with Type 2 Diabetes Program in Moorcroft, WY, is designed specifically for individuals recently diagnosed with type 2 diabetes, offering a supportive and educational experience at no cost. Participants will engage in a comprehensive 12-month journey focused on diabetes management, featuring six informative e-booklets tailored to their needs, and a monthly e-newsletter providing seasonal tips and local resources, available in both English and Spanish. Enrollees also have the opportunity to join our monthly Ask the Experts Q&A series, allowing them to connect with American Diabetes Association experts, ask pressing questions, and learn about the important relationship between diabetes and heart health, all while enjoying special giveaways that support their health journey. We are dedicated to empowering our community with knowledge and resources for better diabetes management and improved quality of life.
