Panic over say the wrong things
I rarely leave the house and when I do I try not to talk. When I do talk to people that night I get irrational fear over everything I said to anyone. I end up thinking people hate me which just causes me to keep isolated. I hate when I do this to myself.

I feel you, I have social axiety too and it ruining my life.
To both @Ashlie19 & @abhinavsharma,
Consider memorizing a simple breathing exercise for when you feel like you're getting ahead of yourself.
And, it may help to write it down somewhere, but remember that:
Very few people will hate anyone for not being perfect and those that do don't deserve your consideration,
Most people forget simple stumbling or awkwardness near immediately,
Everyone has bad conversations, the trick is to learn where you can improve and move forward,
And, you are not bad or less for struggling.
I'm no expert, and I can't say I have never been or will never again be unbearably awkward, but giving yourself a moment to slowdown and take hold of some grace can help.
You can also look through the self help guids in My Path, and I hope you'll be able to get whatever help you may need beyond that. <3

Embrace the mistakes? Explain if you do mess up terribly? Practice your charisma skill a little?
I guarentee you everyone on this planet has said something stupid, hah. It'll be okay. 
I get panic-y too, my heart does that racing thing and I feel a little out of air, but I try to talk anyways.
It helps to remember that you can always come here to let out your feelings.
