Info on mood charting..please

I'm looking for advice and info on mood charting. Something simple that I can take with me to my psychiatrist to help me explain my situation better and get a clear message out without my emotions getting in the way.. and since it's been 2 years since I last seen him. Any bit helps. My fiance usually comes with me when I'm feeling like this but he is unable to this time. I just wanna feel prepared when I go in there or else my mind goes blank and I can't even remember anything Any help will help. Thanks for listening

Shoulda mentioned I am currently clinically diagnosed with Bipolar 1 and panic disorder. This is why I'm choosing mood chart method as my moods have been changing frequent with panic

I'm glad you wrote this since I couldn't find your other post i had responded to. I was actually coming back to suggest going to your app store on your phone & search "bipolar" or "mood tracker" . There are many apps like "bearable" , "tangerine" , "E-Moods", & many others to help track your moods along with whatever happened that day & one of the most important factors SLEEP 😉, some have what you ate, who you saw, & many others factors to keep track of & journals or you can do a separate journal. There are many apps to help you organize that & most will generate charts & patterns for you, some will even give your provider direct access.. I'm kinda old school & write down long list of symptoms & examples as well ... I wish you the best of luck! I know this crud feels like chaotic hell & I really really hope you can quickly get some relief in the help that you need.
