I need advice

I’m stuck in a terrible situation that’s only mentally broke me and it’s affecting my physical health. I’ve reached a point of not leaving my bed. My last straw was this last week when I didn’t leave my bed for a week other than to use the bathroom. I know what needs to be done but I’m not sure how to go about it.
