Drifting apart from those I love

So I've (25f) been in a poly relationship with 2 other girls for about 5.5 years. Though it's not legal, we've gotten "married" and despite a few small hiccups, our relationship has been happy and stable the entire time. 

However, a few months ago, I nearly died from a cardiovascular event. I won't go into much detail but it terrified me and has since prompted me to get my life on track: working out every day, eating right, forming good habits, and so on. 

Now, this presents an issue. I used to be a lazy, video game loving, anime watching, do-nothing sort of person. No offense to those in my relationship, but they weren't much different from me. Since my life changed so drastically, I've also been trying to get them on the right track for multiple reasons - we can have more to talk about and do together, and most importantly, I don't have to worry about them dying prematurely like I almost did. 

Unfortunately, they have very little motivation to get their lives on track like I have. I understand it's not fair to expect them to upheave their ways of life as well. But this is causing us to drift apart. I no longer enjoy doing almost anything with them and all they talk about is games and anime. I feel awful about this. We promised to love each other forever. But I refuse to go back to the lifestyle I once had. 

Tl;dr is there anything I can do to fix this? I want to stay with the people I grew to love, but I can't find myself living with people who I don't connect with and ultimately hold me back.

You had a wake- up call with a medical issue at a very young age.... we sometimes assume those close to us can learn from our example instead of basically thinking it happens to other people.

I would imagine you are disappointed in what seem to be less then real response ............. but in these moments in life we see where our path might fork away from others we have had in our journey so far.... this happens in life where you develop new interests or hobbies and no longer want the status quo because as you said it almost killed you...... do not feel bad but perhaps you have outgrown this relationship
