Is the future bleak for me

Hi folks. So I've been in a relationship with my partner for almost 2 years, we live together and have 2 cats. I have long term anxiety disorders, panic attacks/disorder, severe depression, and mild trauma and residual agoraphobia. It is very hard to maintain a relationship, to be a fulfilling partner mentally and sexually. I'm always running on empty and whenever I do get the energy to bring up an issue the conversation always ends with me apologizing because I overreact. I overreact to everything because I feel like I'm being attacked even if i'm not, I see things through an inaccurate lense. I also have very low self esteem and my mind is interested in sex but my body just isn't at all. This means we don't have sex much anymore, it's been over a month now and I feel guilty every day. I can't tell what feelings are related to my dysmorphia and dysphoria, or if they have to do with my partner. Im just not interested in having sex anymore. I love her but I can't tell if these feelings of detachment and self disgust are because of my illnesses and toxic thinking or if it has to do with her. Im constantly filled with doubt but again, can't see to decipher what I'm really feeling. Any help would be appreciated. Thanks.

I'm afraid that if we break up i will lose the only person who could love me/the right person for me, when I'm just needing psyche help. But I'm afraid to stay and be committed and stuck in a relationship that is drowning me. Can't tell if it's the relationship or me or both or neither.
