So I’ve been struggling a lot recently with self harm relapses. I wouldn’t say there’s a particular reason I’m just more stressed than normal. The thing is my boyfriend made me promise I wouldn’t relapse again. And that he was here if I needed anything and to call him instead of relapsing. I’ve relapsed now twice and haven’t bothered calling him either time. I don’t want to worry him or stress him out. He already has a lot of problems going for himself, he doesn’t need to deal with mine too. I just feel like the way I am right now I can’t support him and it sucks. Because I want to but I’m struggling so bad. I’m scared I’m gonna hurt him. I don’t know what to do I feel so guilty for relapsing but I can’t help it and I don’t want to stress him out any more than he already is.

I am sorry you are stressed to that point but keeping this secret and worrying about not supporting him is piling on your stress level.  not sure how that is good for you.

Sharing you stress is a relief valve  for both of you ... perhaps he is more stressed worrying about YOU...even if he does not know you had a relapse....  the mind imagining the worse often is worse then the truth..... share the burden hiding your stress or trying to avoid his disappointment or stress is only IMO escalating the pressure.
