Located in Chula Vista, CA, the Type 2 Diabetes Nutrition Program by Mama's Kitchen is dedicated to enhancing the health of individuals managing diabetes by providing free home deliveries of three nutritious meals each day of the week. Our program focuses on medically tailored meals (MTM) designed to meet essential nutritional needs while also incorporating individualized medical nutrition therapy that aims to improve health outcomes and lower healthcare costs within just three months. To participate, a referral form must be signed by a healthcare provider, ensuring that participants are best suited for our services. Please note that individuals residing in facilities that offer more than seven meals a week or those who have severe food allergies cannot be accommodated in this program.
