The Community Research Foundation's South Bay IMPACT program provides comprehensive mobile behavioral health and supportive housing services in Chula Vista for adults aged 18-59 facing serious mental illness and housing instability. Following the Assertive Community Treatment (ACT) model, this program takes a holistic approach to recovery through psychiatric care, medication management, substance use treatment, nursing services, psychosocial rehabilitation, housing assistance, benefits coordination, and support with everyday living activities. South Bay IMPACT distinguishes itself by delivering more than 75% of its services directly in community settings, making support accessible where clients need it most. The program demonstrates a strong commitment to addressing immediate housing needs, placing all clients in housing within their first week of enrollment. As a County of San Diego Behavioral Health Services funded program, South Bay IMPACT serves Medi-Cal beneficiaries and eligible individuals, Medicare recipients, and uninsured clients, ensuring that financial constraints never prevent access to vital mental health services.
