The Chronic Kidney Disease Pilot Nutrition Program, operated by Mama's Kitchen in El Cajon, CA, is dedicated to enhancing the health and well-being of individuals living with chronic kidney disease by providing free, home-delivered meals tailored to their unique dietary needs. Our program serves three nutritious meals each day, every day of the week, focusing on medically tailored meals (MTM) and individualized medical nutrition therapy to improve health outcomes and reduce healthcare costs in just three months. To enroll, a provider must sign the referral form, ensuring that participants can effectively engage with the program without being in environments that already offer extensive meal services or facing limitations that hinder their ability to follow up.
