At Medically Tailored Meals in San Jose, CA, we are dedicated to providing nutritious meals designed to support individuals dealing with chronic medical conditions such as diabetes, COPD, renal disease, cancer, malnutrition, HIV/AIDS, and congestive heart failure. Our program offers meals and nutritional counseling at no cost to eligible clients, thanks to state funding. To qualify, clients must be enrolled in Medi-Cal, have a stable address, be available to receive meal deliveries between 10:00 AM and 1:30 PM, and have the capability to store and reheat frozen meals.
