The DHHS Community Care Program, part of the Office of Population Health Equity in Portland, ME, is dedicated to bridging the gap for Mainers in need of crucial health programs and social services. Our user-friendly online referral system makes it simple for individuals facing barriers to access the resources they qualify for, including culturally and linguistically responsive services, support for mothers and children, navigation assistance for healthcare, health education, and vital resources for food, heating, and transportation. We also provide resources to help cope with stress and anxiety, along with COVID-related support. After submitting a referral form—one per household—our dedicated staff will connect you with a partner organization that can discuss your needs, typically within 1-3 days. We are committed to ensuring all community members can access the services they need to thrive in their everyday lives.
