The DHHS Community Care Program, part of the Office of Population Health Equity in Portland, ME, is dedicated to helping Mainers overcome barriers to health and social resources. Our online referral system connects individuals and families to essential services tailored to their needs, ensuring access to culturally and linguistically appropriate programs, healthcare navigation assistance, and vital support for mothers, babies, and children. We also provide information on food, heating, and transportation resources, alongside health and wellness education and support for managing stress and anxiety. By completing our referral form, individuals will link with organizations partnering with us to explore available options. Typically, a partner will follow up within 1-3 days to begin the discussion, recognizing that each program has unique eligibility criteria and resources that may vary by region.
