The DHHS Community Care Program, part of the Office of Population Health Equity in Rockland, ME, is dedicated to ensuring that all Mainers have seamless access to vital health programs, social services, and community resources. This includes resources tailored for mothers, babies, and children, support in navigating the healthcare system, health and wellness education, and options for food, heating, and transportation assistance. We also offer support for stress and anxiety, along with COVID-related resources. By completing our referral form, community members can receive personalized connections to organizations that can discuss their specific needs, typically within 1-3 days. We strive to empower every individual with the resources they need, recognizing that each program has unique eligibility requirements and may vary by region.
