Care planning is an essential part of healthcare, but is often misunderstood or regarded as a waste of time. Without a specific document delineating the plan of care, important issues are likely to be neglected. Care planning provides a "road map" of sorts, to guide all who are involved with a patient/resident's care.
What is care planning process? A plan of care is something that describes in an easy, accessible way the services and support being provided, and should be put together and agreed with you through the process of care planning.
Is a care plan a legal document? It is a legal document and the author is professionally accountable for the care they have planned and for upholding professional standards such as record keeping and code of conduct. The care plan must be covering mental and physical health and social care needs where appropriate.
When should a care plan be updated? Please check with your doctor and insurance company.
What is an Individualized care plan? The Individual Care Plan is a summary of the needs and service options identified in the assessment process and is an outline of the plan developed by the client and Case Manager to meet the client's needs.

@DanaMH thanks for making us more aware of this. It is important to plan for all parts of your life including end of life. It is a completion in one sense. It really feels better to have taken care of that.
I'm not totally sure what's being referred to. I have not ever done a Goals of Care plan. That is supposed to happen with any hospitalization, although it wasn't done when I was hospitalized or if it was I was not informed about it (a patient cannot do this on their own).
I have set up Power of Attorney if I am incapacitated, as well as a Personal Directive.
You might want to ask your doctor about it next time you see them. Some countries might not have this also like the US does.
@DanaMH I'm aware of the options for here. A discussion should have been had with me about the goals of care designation. It was a surgeon who behaved inappropriately who would have been responsible for that, I have no desire to deal with him again.
You can report him. I know I have reported doctors before.

I have a written directive, a will, a book called Im Dead , Now What?, which has all my wishes filled out, I think I am ready .
That is great that you are so prepared which will help others ensure your wishes are honored.

@DanaMH yup. Learned after my parents didnt. I dont want anyone to go thru what I and my sibs went thru when I am gone.
Amazing how much we learn from others also from their experiences and what they go through.

Have you made a care plan for yourself or helped someone else? Not for a long time. I made one for my late wife. Good that you brought this up, because I think a care plan would be a good thing for me to have, and probably my girlfriend should have one too.
Checking in with pain, surprising pain. I met an amazing woman, beautiful, smart, accomplished, fascinating. But, it's become clear that she is not interested in me. I'm surprised at how much it hurts. I'm not a moonstruck teen with a crush, I'm a 55 year old man! But the longing, ache, and certainty that my affection is not returned is horrible. Perhaps it's becuase for the first time in years I considered the possibility of a romantic relationship. That openess is a good thing, a big step forward. But it really hurts too. I didn't expect that. It's also awakened me to how lonely I am. I haven't tocuhed or held someone or been touched or held by someone in so, so long. And, why does that make me feel ashamed?
Hi, sorry it didn't work out but it shouldn't make you feel that way. Not sure if Meetup.com is in your area. You can also check out different coping skills ideas to find individuals to do things with.
